During perimenopause, testosterone levels fall - sometimes earlier than oestrogen. This drop can contribute to low motivation, fatigue, reduced libido, and blunted mood.
In certain cases, low-dose testosterone replacement (as part of Menopausal Hormone Therapy) can improve energy, concentration, and depressive symptoms when oestrogen alone isn’t enough.

Evidence from the 2021 International Menopause Society consensus supports its cautious use for hypoactive sexual desire and emerging mood benefit in perimenopausal women.
